Zinc
An essential mineral with a test that works, provided you know what it is sensitive to.
Plasma or serum zinc is the standard blood test for zinc status, typically reported as roughly 70 to 120 mcg/dL. Systematic review evidence confirms it responds dose-dependently to dietary zinc and is a reliable biomarker in healthy individuals, while expert panel guidance notes it is also influenced by recent meals, time of day, inflammation and certain medications. A fasting morning draw, read alongside an inflammation marker, gives the most interpretable result.
What it actually measures
Zinc does not get a headline the way iron or vitamin D3 do, which is odd given the size of its job.
It sits inside somewhere in the region of three hundred enzymes. It holds the structural shape of the proteins that read your DNA. It is required for immune cells to function, for wounds to close, for taste and smell to work, for testosterone production, and for the enzymes that convert T4 into T3 in your thyroid pathway.
Here is the part that shapes the whole test though. Your body stores very little zinc in any accessible way. There is no zinc equivalent of ferritin, no warehouse you can measure. What zinc you have is largely locked into working tissue, doing its job.
Which means plasma zinc, the fraction circulating in your blood, is a small slice of a total that is mostly held elsewhere. Around one percent of body zinc, give or take.
That is not a reason to dismiss the test. It is the reason the test behaves the way it does, and it explains why collection conditions matter here more than for most markers.
Normal and optimal are different questions
I want to handle this marker slightly differently from the others, because the interesting question is not whether the range is too wide. It is what the test is genuinely good at.
The evidence base here is unusually clear. A 2009 systematic review in the American Journal of Clinical Nutrition took on exactly this question, working through 32 potential zinc biomarkers across 46 publications with formal inclusion criteria and meta-analysis.
What it found: plasma zinc responded in a dose-dependent way to dietary manipulation in adults, in men and women, in pregnancy and lactation, in the elderly, and in people at both low and moderate baseline zinc status. Urinary zinc responded across every subgroup with sufficient data. Hair zinc responded too.
It also found what does not work, which is just as useful. Red cell zinc, platelet zinc, mononuclear cell zinc and alkaline phosphatase activity did not appear to be effective biomarkers of zinc status. So if someone offers you one of those as a better test, the systematic review already looked.
The review concluded plainly that plasma, urinary and hair zinc are reliable biomarkers of zinc status in healthy individuals.
Then in 2015 an expert panel in the Journal of Nutrition added the operating instructions. It recommended three measures together, dietary zinc intake, plasma zinc concentration, and height for age in growing children, and it named the specific things that move plasma zinc independent of status: recent meals, time of day, inflammation, and certain drugs and hormones.
Put those two together and you get a fair picture. This is a validated test with known sensitivities, which is a very different thing from an unreliable one. You just have to control the conditions.
The two things that will fool you
So. Two specific traps, and both are easy to avoid once you know they exist.
The first is your last meal. Plasma zinc drops after eating and follows a daily rhythm, which means a mid-afternoon draw after lunch is measuring something other than what you wanted. Fasting, in the morning, in a trace-element tube. Ask when the appointment is made rather than discovering it afterward.
The second is inflammation, and it is the bigger one. During an inflammatory response your body pulls zinc out of circulation and into tissues. It is a deliberate move, part of how the immune system limits what a pathogen can get hold of.
But the blood test does not know why the number fell. So an infection, a recent injury, or any chronic inflammatory process can produce a low plasma zinc in someone whose total body zinc is perfectly adequate.
If that pattern sounds familiar, it should. It is the mirror image of what inflammation does to ferritin, where a raised value can hide an empty tank. Same underlying phenomenon, opposite direction, same solution.
Run hs-CRP on the same draw. It is inexpensive, and it turns an ambiguous zinc result into an interpretable one. Low zinc with a clean CRP means something quite different from low zinc with inflammation running.
What moves it
Down: low intake, particularly on a diet built mainly on grains and legumes, since phytate binds zinc and reduces absorption. Malabsorption from coeliac disease, inflammatory bowel disease or bariatric surgery. Heavy alcohol intake, which increases losses. Chronic diarrhoea. Some diuretics and long term proton pump inhibitors. High dose iron supplements taken at the same time, which compete for absorption. Pregnancy and lactation, through demand. And inflammation, which lowers the blood value without lowering body zinc.
Up: oysters, which contain more zinc by a wide margin than any other food, then red meat, poultry, shellfish, pumpkin seeds, cashews, chickpeas and lentils. Soaking, sprouting and fermenting plant sources, which breaks down phytate and improves absorption meaningfully. And supplementation, which raises it readily and is where the copper caution below applies.
The honest hedge

The caution that matters most on this page is not about the test, it is about what people do after it. Zinc and copper compete for the same absorption pathway, and sustained high dose zinc supplementation can produce a genuine copper deficiency.
That is not a theoretical concern and it is not a small one. A 2005 report in the American Journal of Clinical Pathology described three cases of zinc-induced copper deficiency initially recognised on bone marrow examination, and a 2023 case report in Cureus documented it presenting as neurological deficit and anaemia. Those are real people, found late, because nobody was looking at copper while the zinc was going in.
So the rule I would give plainly: zinc supplementation at any meaningful dose deserves a duration, a reason, and copper in the conversation. It is not a supplement to take indefinitely because it seemed like a good idea during a cold.
On the test itself, I would rather you knew its sensitivities than either trusted it blindly or wrote it off. Fasting, morning, with an inflammation marker alongside, it does the job the systematic review says it does.
I am not saying you are short of zinc. I am saying it is a variable, one that touches immunity, thyroid conversion, wound healing and hormone production all at once, and one that no standard panel ever measures.
So. What to actually do.
Tonight, at no cost. Check whether you are already taking zinc, and add up every source. It appears in multivitamins, immune formulas, prostate supplements and lozenges, and people routinely take three products containing it without ever totalling the dose.
Look at your plate rather than your cupboard first. Oysters, red meat, shellfish, pumpkin seeds, cashews, chickpeas, lentils. If plant sources are the bulk of your intake, soaking and sprouting genuinely improves how much you absorb.
When you test, control the conditions. Fasting, morning, trace-element tube. Ask when you book, because zinc is more sensitive to collection than most tests.
Order an inflammation marker alongside. hs-CRP is what tells you whether a low zinc reflects your status or an inflammatory response, and it is inexpensive.

Take the result to someone who reads it in context. A good functional medicine doctor will read a zinc value against your intake, your inflammation and your symptoms rather than against the cutoff alone, which is exactly how the expert panel recommends it be used.
None of this has to happen this week. But tonight you can add up how much zinc you are already taking.
You are not running low on one mineral in isolation. You are a system where zinc holds the shape of the proteins that read your genes, and copper sits right beside it competing for the same door.
Your body is not broken. It is blocked. And sometimes the block is a trace mineral that never appears on a standard panel, quietly limiting immunity, thyroid conversion and repair all at once.
