A vitamin D test measures 25-hydroxy vitamin D, the storage form that reflects your actual status over recent weeks. It is the correct test for assessing vitamin D3, and it is not usually included in a standard annual panel unless someone asks for it.
What this test actually measures
There are two vitamin D numbers a lab can run, and only one of them answers the question you are asking.
25-hydroxy vitamin D is the storage form. It is what your liver makes after you get sun on your skin or take D3, and it sits in circulation with a half-life measured in weeks. That stability is exactly what makes it useful, because it reflects your status rather than what you did yesterday.
The other one, 1,25-dihydroxy vitamin D, is the active hormone form. It is tightly regulated, it moves fast, and in someone with low stores it can read perfectly normal while the storage form is on the floor. Ordering it to assess vitamin D status is a well known mistake. It has real uses, in specific kidney and calcium conditions, but general status is not one of them.
This test is the 25-hydroxy one. If you are checking your vitamin D3, this is the correct test.
One thing worth knowing about what a total means. The 25-hydroxy result is usually reported as a total that adds two chemically distinct forms together, 25(OH)D3 and 25(OH)D2. That matters if you take vitamin D2, since a 2024 dose-response meta-analysis in Steroids found D2 supplementation raised total vitamin D while decreasing 25(OH)D3 concentrations by 4.63 ng/mL. A total can therefore look acceptable while the D3 fraction is falling underneath it, and only the fractions show that.
What the number means, and where the guidelines just moved
Here is where I have to be more careful than most pages on this topic are, because the ground genuinely shifted and a lot of writing has not caught up.
For over a decade the reference point most people worked from came from the 2011 Endocrine Society clinical practice guideline, which set out deficiency below 20 ng/mL, insufficiency from 20 to 30, and sufficiency at 30 and above. Those numbers are the reason get above 30 became the standard advice, and they are still what a great many practitioners and websites quote today.
In 2024 the Endocrine Society published a guideline communication stating that it no longer endorses its previously proposed definitions of sufficiency at or above 30, or insufficiency between 20 and 30. The stated reasoning is that in generally healthy populations, the available clinical trial evidence does not support assigning specific 25-hydroxy vitamin D thresholds that predict who benefits from supplementation.
Read that carefully, because it is easy to misread in both directions.
It does not say vitamin D does not matter. It does not say deficiency is not real. What it says is narrower and more honest: the trials do not support drawing a confident line and declaring that everyone above it is fine and everyone below it should supplement.
I will be honest with you, because I always will be. I think this is one of the more interesting corrections in nutritional medicine in recent years, and it is uncomfortable for a wellness industry that has been quoting 30 as settled for a decade. The number on your result is real. What it means for you specifically is less settled than almost anyone selling you something will admit.
What has not changed: genuinely low values, the ones well below 20, are associated with real bone and musculoskeletal consequences and are worth addressing. What has changed: the confident middle, where a result of 28 gets called insufficient and triggers a supplement recommendation, has considerably less evidence behind it than the last decade of advice implied.
That is the honest state of it. Test, know your number, and have the conversation with someone who can weigh it against the rest of you.

How the pricing works
I want to be straightforward about this, because lab pricing in the wellness industry frequently is not.
Jess takes no markup on the test itself. The lab cost is passed through exactly as it comes. What you pay is the test cost, the dispensary's service fee, the authorization fee, the draw fee, a flat handling fee for placing and managing the order, and the card processing cost. The handling fee is the part that is Jess's, and it is flat regardless of how expensive the test is.
That structure is deliberate. A percentage markup on labs creates an incentive to recommend expensive tests, and this way there is not one.
Because several tests share one draw fee, adding a second or third test to the same order costs considerably less than ordering them separately. Who this test is worth running for
Anyone who has never had it run, honestly, since it is not on a standard panel and most people have genuinely never measured it.
More specifically: if you live at a northern latitude, if you are testing in winter, if you spend most daylight hours indoors, if you have darker skin, which increases the sun exposure needed to make the same amount, if you have a gut condition affecting fat absorption, since D is fat soluble, or if you are already supplementing D3 and have never checked where that has put you.
That last one matters more than people expect. Vitamin D is fat soluble, which means it accumulates, and it is possible to take too much over a long enough period. If you have been taking a high dose for years on the strength of general advice rather than a number, measuring is the responsible move in both directions.
Ordering restrictions
The lab network cannot issue orders to residents of New York, New Jersey, Rhode Island, or Hawaii. New York, New Jersey, and Rhode Island prohibit direct-to-consumer lab testing outright, requiring a treating provider's signature on every requisition. Hawaii is excluded by the network. This is tied to where you live, not where you get drawn, so it applies to blood draws as well as kits.

What to do with the result
If it comes back genuinely low, well under 20, that is worth acting on with a provider, and it is also worth asking why. Absorption, sun exposure, and body composition all affect it, and a supplement addresses the number without addressing the reason.
If it comes back in the middle, somewhere in the twenties or low thirties, this is exactly the zone the 2024 communication was talking about, and the honest answer is that the evidence does not confidently tell you what to do. Sun exposure, food, and a conversation beat a confident internet recommendation.
If it comes back high, and especially if you have been supplementing, that is a real finding worth bringing to your doctor rather than ignoring because high vitamin D sounds harmless.
If you supplement, use D3 rather than D2. Cholecalciferol on the label, not ergocalciferol. D3 raises total 25(OH)D more effectively and, unlike D2, does not push down the 25(OH)D3 fraction. If your D2 was prescribed, raise the question with your prescriber rather than switching on your own.
In every case, retest rather than assume. If you change anything, three months is a reasonable interval to see where it landed, since the storage form moves slowly.
