Calcium and Thyroid Medication
A four-hour spacing question, and one of the better documented interactions on this site.
Evidence: Well documented
Calcium supplements reduce the absorption of levothyroxine when taken at the same time. A prospective study in JAMA found free T4 and total T4 fell significantly during three months of concurrent calcium carbonate and rose again after it was stopped, and a later study found calcium carbonate, citrate and acetate all reduced levothyroxine absorption by roughly 20 to 25 percent. Separating the doses by about four hours is the standard practical answer.
The mechanism, in plain terms
Levothyroxine is thyroid hormone taken as a tablet, and it has a narrow window in which it gets absorbed and a genuine sensitivity to what else is in your stomach.
Calcium interferes with it directly. The proposed mechanism is that thyroxine binds, or adsorbs, to calcium carbonate in the acidic environment of the stomach, and hormone that is stuck to a calcium particle does not cross into you.
The original JAMA study supported this with an in vitro component alongside the clinical one, showing thyroxine binding to calcium carbonate.
The important feature of that mechanism is that it requires the two to be present together. It is physical binding in the stomach rather than something calcium does to your thyroid or to hormone already in circulation.
Which is why the answer here is timing rather than avoidance, and why the timing works.
What actually happens to people
This is one of the better documented interactions in this whole set, so I can be specific.
A prospective study published in JAMA in 2000 followed twenty patients with hypothyroidism on stable long-term levothyroxine, all with normal free T4 and thyrotropin at baseline. They took 1200 mg of elemental calcium as calcium carbonate together with their levothyroxine for three months, with measurements at baseline, at two and three months, and again two months after stopping the calcium.
Mean free T4 and total T4 both fell significantly during the calcium period and rose again after it was discontinued. Free T4 went from 17 pmol/L at baseline to 15 during calcium and back to 18 afterwards, with an overall p less than 0.001.
That is about as clean a demonstration as you get: the effect appeared with the calcium, and it reversed when the calcium stopped.
A 2011 study in Thyroid then asked whether it was specific to calcium carbonate, since people frequently take other forms. It ran single-dose pharmacokinetic studies in eight healthy euthyroid adults, comparing levothyroxine alone against levothyroxine with calcium carbonate, calcium citrate, or calcium acetate, each providing 500 mg of elemental calcium.
All three reduced levothyroxine absorption by about 20 to 25 percent. The authors noted this contradicted a prior report suggesting calcium acetate had little effect, and concluded that hypothyroid patients should be cautioned to take levothyroxine well separated from all of these calcium formulations.
So switching calcium form does not solve it. Separating the doses does.
The doses and durations that show up
The studies used 1200 mg of elemental calcium in the JAMA work and 500 mg in the form-comparison study, and both showed an effect, so this is not confined to unusually large doses.
The relevant variable is proximity in time, not the size of the calcium dose or how long you have been taking it. The interaction happens dose by dose, in the stomach, each time they coincide.
That has a practical consequence worth stating: someone who has been taking both together for years is not accumulating a problem, they are simply absorbing less of each levothyroxine dose than their prescriber assumes. Which is a dosing accuracy problem rather than a toxicity one.
The conventional recommendation is to separate them by about four hours. Levothyroxine is usually taken on an empty stomach in the morning, which makes calcium at lunch or in the evening a straightforward arrangement.
Worth knowing that calcium is not the only offender. Iron supplements, some antacids, and certain other products interfere with levothyroxine absorption too, which is why the general advice is to take levothyroxine alone and separated from everything.
The honest hedge
I am not a doctor and I am not diagnosing anyone, and the specific thing I want to be careful about here is what you do with this information.
Do not change your levothyroxine dose because of this page. And do not stop your calcium without telling anyone. Either of those, done quietly, is how you turn a manageable timing issue into an actual problem.
The reason is this. If you have been taking them together and your dose was titrated in that state, your prescriber effectively adjusted your dose to account for reduced absorption without knowing it. Separating them suddenly increases how much hormone you absorb from the same tablet, which can push you toward over-replacement.
So the correct move is to separate the doses and tell your prescriber you have done so, so they can re-check your levels on the usual six to eight week rhythm.
That timescale matters too. T4 has a half-life of about a week, so levels take weeks to stabilise after any change and testing sooner mostly measures noise.
This is genuinely good news overall. A well documented interaction with a free fix is a much better situation than most things on this site.
So. What to actually do.
Tonight, at no cost. Work out whether your calcium and your levothyroxine currently go down at the same time, including calcium hiding inside a multivitamin or an antacid.
Separate them by about four hours. Levothyroxine on an empty stomach in the morning, calcium at lunch or evening. That is the whole fix.
Tell your prescriber you have made the change. This is the part people skip and it is the part that matters, because your dose may have been titrated around the reduced absorption.
Do not adjust your own dose. That belongs with your prescriber, on a six to eight week rhythm, because T4 takes weeks to stabilise.
Check what else goes down with it. Iron supplements and some antacids interfere as well, so levothyroxine alone and separated from everything is the general rule.
Have the levels checked properly when you do. A full thyroid panel with free T4 and free T3 tells you more than a TSH alone, particularly around a change like this.
None of this has to happen this week. But tonight you can check whether the two bottles go down together.
You are not failing your medication. You are absorbing less of it than anyone realised, because something else was in the stomach at the same moment.
Your body is not broken. It is blocked. And this is one of the rare cases where the block is genuinely, cheaply, completely solved by moving one bottle four hours later.
