An advanced lipid panel adds apolipoprotein B and inflammation markers to the standard cholesterol numbers. ApoB counts the atherogenic particles directly, at one molecule per particle, while standard LDL cholesterol measures the cholesterol carried inside them, which varies. A 2025 systematic review of 15 discordance studies covering 593,354 participants found apoB outperformed LDL cholesterol in 9 of 9 comparisons.
What this test actually measures
Picture the particles that carry cholesterol around your body as delivery vans, and picture a road where some of them occasionally get stuck in the wall and start a slow pile-up that becomes plaque.
A standard lipid panel measures the cargo. LDL cholesterol is the amount of cholesterol carried inside your LDL particles, added up, which is essentially the weight of what is being shipped.
An advanced panel adds a count of the vans. Every atherogenic particle carries exactly one apolipoprotein B molecule on its surface, one particle to one apoB, no exceptions, so measuring apoB counts particles directly.
That matters because a particle enters an artery wall as a particle, regardless of how full it happens to be.
This panel also includes inflammation markers, which is the other half of the same story: particle count is what accumulates, and inflammation is a large part of what turns accumulation into damage.
What the extra numbers actually add
Your LDL cholesterol and your apoB usually agree. When they do, either one tells you roughly what you need to know.
The interesting question is what happens when they disagree, which researchers call discordance, and there is a clever reason to study exactly that. The two markers are so tightly correlated that ordinary statistics struggle to separate them, so discordance analysis isolates the people in whom they point different directions and asks which one was right.
A 2025 systematic review in the Journal of Clinical Lipidology gathered every such study it could find: fifteen studies, 593,354 participants, populations on and off statin therapy, four different analytical approaches.
ApoB outperformed LDL cholesterol in 9 of 9 comparisons. Against non-HDL cholesterol, apoB came out significantly more accurate in 7 studies, and the authors concluded that neither LDL-C nor non-HDL-C is an adequate clinical surrogate for apoB.
In the interest of giving you the whole picture, a 2026 Copenhagen General Population Study analysis in JAMA Cardiology, following 94,398 people for a median of over thirteen years, found non-HDL cholesterol and apoB associated with similar risk on continuous scales. So the specific comparison between those two is still live. The comparison people actually care about, apoB against the LDL cholesterol on their existing report, is where the evidence is consistent.
Here is the practical part. Discordance is not random. It concentrates in people with insulin resistance, high triglycerides, metabolic syndrome and type 2 diabetes, because those conditions produce more particles carrying less cholesterol each. Which means a reassuring LDL cholesterol is least trustworthy in exactly the group most likely to be told their cholesterol is fine.
On the inflammation side, hs-CRP has its own evidence. The JUPITER trial in 2008 randomised people with raised hs-CRP but not raised LDL to rosuvastatin or placebo and found a reduction in vascular events, which is to say inflammation identified people cholesterol alone did not.

How the pricing works

Who this test is worth running for
Anyone with a family history of early heart disease, and anyone who has been told their cholesterol is fine but has other metabolic signs.
Specifically worth it if your triglyceride to HDL ratio is above about 2.5, which you can calculate free from a lipid panel already in your records. A high ratio is the clearest flag that your LDL cholesterol may be understating your particle count.
Also worth it for anyone with insulin resistance, type 2 diabetes, polycystic ovary syndrome or excess weight around the middle, since those are precisely the conditions that produce discordance. And for anyone already on lipid-lowering treatment who wants to know whether particle count actually came down rather than only the cholesterol figure.
Ordering restrictions
The lab network cannot issue orders to residents of New York, New Jersey, Rhode Island, or Hawaii. New York, New Jersey, and Rhode Island prohibit direct-to-consumer lab testing outright, requiring a treating provider's signature on every requisition. Hawaii is excluded by the network. This is tied to where you live, not where the sample is collected, so it applies to kits as well as draws.

What to do with the result
If apoB is raised, the levers are the same ones that lower LDL. Reducing saturated fat, increasing soluble fibre, losing excess visceral fat, and improving insulin sensitivity through exercise and sleep. Where medication is appropriate, that is a prescribing decision.
If apoB is raised while LDL cholesterol looked fine, that is exactly the discordance this panel exists to reveal, and it is the good version of this result because you found it.
If inflammation markers are raised, repeat before acting, since any recent infection, injury or hard training raises them temporarily. Persistently raised is the finding that matters.
Take it into a full risk conversation rather than treating a number. Blood pressure, blood sugar, family history and possibly imaging all belong in that discussion, and a blood test does not tell you whether plaque is already present.
