Ferritin and Iron Panel
Stores and transport in one draw, because either one alone can point the wrong way.
This panel measures ferritin, which reflects stored iron, alongside serum iron and total iron binding capacity, which describe how much iron is in transit and how much carrying capacity is available. Ferritin is the marker that falls first in iron deficiency, and it is not included on a standard blood count, which is why iron stores can be genuinely empty while a blood count still reads normal.
What this test actually measures
Think of iron in your body as having two states: what is stored, and what is moving.
Ferritin is the storage protein. It is your reserve, the iron sitting in your liver, spleen and bone marrow waiting to be used, and a ferritin measurement is the closest thing you have to a fuel gauge on that tank.
Serum iron is what is in transit through your blood right now, and total iron binding capacity is how much carrying capacity your transport proteins have. Divide the first by the second and you get transferrin saturation, which describes how full the transport system is running.
Each answers a different question, and any one of them alone can genuinely point the wrong way. Serum iron swings with your last meal. Ferritin rises with inflammation regardless of your iron. Saturation is steadier than serum iron but says nothing about the size of the reserve.
Which is exactly why they are ordered together. Read as a set, they tell you both how much iron you are carrying and whether supply is reaching the system.
What the numbers mean, and the one thing that distorts them
Most labs draw the ferritin reference range starting somewhere between 12 and 15 ng/mL at the bottom. That figure is not a health target, it is roughly the point at which iron deficiency anaemia becomes likely, and a great many people sit above it while feeling the effects of depleted stores.
That gap is where the research on this marker actually lives. A 2003 randomised controlled trial published in the BMJ tested iron supplementation specifically in non-anaemic women with unexplained fatigue, and a 2020 randomised placebo-controlled trial in Scientific Reports looked at intravenous iron in non-anaemic blood donors with iron deficiency, measuring fatigue and general health.
Notice what both of those have in common. The participants were not anaemic. Researchers went looking in exactly the space between an empty tank and a low blood count, because that space is real.
Now the thing that distorts the number, and it is the single most important interpretive point here. Ferritin is an acute phase reactant, meaning it rises during inflammation regardless of how much iron you have. So an infection, an autoimmune condition, obesity or any chronic inflammatory process can lift a ferritin result on top of an empty tank.
That is why an inflammation marker belongs in the same draw. hs-CRP is inexpensive and it is what tells you whether a comfortable ferritin can be trusted.
It is also where transferrin saturation earns its place. A 2019 systematic review in Critical Reviews in Clinical Laboratory Sciences worked through 41 publications and concluded saturation is useful alongside ferritin for diagnosing iron deficiency, and specifically valuable as a first-line test in people with chronic inflammatory disease, which is precisely the group in whom ferritin misleads.

How the pricing works
I want to be straightforward about this, because lab pricing in the wellness industry frequently is not.

Who this test is worth running for
Anyone who has been told their blood count is normal and is still tired. That is the single most common reason this panel is worth running, because a complete blood count measures hemoglobin, and hemoglobin is the last thing your body lets fall.
More specifically: anyone with heavy periods, anyone who has been pregnant recently, anyone donating blood regularly, endurance athletes, people eating little or no meat, anyone with coeliac disease or another absorption problem, and anyone with unexplained hair shedding, restless legs, or breathlessness on stairs.
It is also worth running before supplementing rather than after. Iron is one of the few nutrients where guessing carries genuine downside, since your body has no efficient way to shed an excess, and a high transferrin saturation is a real finding that needs a doctor rather than more iron.
Ordering restrictions
The lab network cannot issue orders to residents of New York, New Jersey, Rhode Island, or Hawaii. New York, New Jersey, and Rhode Island prohibit direct-to-consumer lab testing outright, requiring a treating provider's signature on every requisition. Hawaii is excluded by the network. This is tied to where you live, not where the sample is collected, so it applies to kits as well as draws.

What to do with the result
If ferritin comes back low, that is worth acting on with a provider, and it is worth asking why rather than only what to take. Iron deficiency in an adult is a finding that should prompt a search for a cause, and heavy periods, coeliac disease and gastrointestinal blood loss are the ones worth ruling out properly.
If ferritin looks comfortable but hs-CRP is raised, treat the ferritin with caution. That combination is exactly the situation where transferrin saturation carries more information than the storage number does.
If transferrin saturation comes back high, particularly above 45 to 50 percent across more than one test, that belongs in front of a doctor. Iron overload including hereditary hemochromatosis is screened for this way, and it accumulates quietly over decades.
In every case, correct with a provider setting dose and duration. Then retest after about three months, since rebuilding stores is slow and stopping as soon as a blood count normalises tends to land you back where you started.
