Vitamin A Test
A fat soluble vitamin with a real ceiling, which makes measuring it worthwhile in both directions.
This test measures serum retinol, the circulating form of vitamin A. Because vitamin A is fat soluble and stored in the liver, it accumulates rather than being flushed out, which means excess is a genuine possibility rather than a theoretical one. Serum retinol is also tightly regulated and falls late, so a normal result does not fully rule out low stores, while inflammation lowers it independently of status.
What this test actually measures
Vitamin A does several jobs that have little obvious connection to each other, which is part of why a shortfall presents in scattered ways.
It is essential for vision, particularly in low light, because retinal, a form of vitamin A, is part of the light-detecting pigment in your eye. It is required for immune function, for skin and mucous membrane integrity, and for cellular differentiation, which is the process by which cells become specialised.
It comes in two dietary forms. Preformed vitamin A, meaning retinol and its esters, from liver, dairy, eggs and fish. And provitamin A carotenoids such as beta-carotene from plants, which your body converts, with meaningful variation between people in how efficiently that conversion runs.

The defining property for anyone interpreting a result is that vitamin A is fat soluble. It is stored in the liver rather than excreted when you have plenty, which means it accumulates over time. That is the reason excess is a real clinical concern here in a way it is not for water soluble vitamins.
Two things that make this number harder to read than it looks
The first is homeostatic control. Your body works to hold serum retinol steady, drawing on liver stores to maintain the circulating level.
The practical consequence is that serum retinol falls late. It generally stays in range until liver stores are substantially depleted, so a normal result tells you the regulation is still working rather than that your stores are full. That is the same logic as hemoglobin holding steady while iron stores empty.
The second is inflammation, and it is the more common trap. Retinol travels bound to retinol binding protein, and retinol binding protein behaves as a negative acute phase reactant, meaning it falls during an inflammatory response.
So an infection, an injury or a chronic inflammatory condition lowers serum retinol independently of your actual vitamin A status. A paper in the Annals of Clinical Biochemistry examining nutritional indicators during an acute phase response works through exactly this class of problem, where inflammation distorts a nutritional marker.
That is why running hs-CRP in the same draw is worth the small extra cost. A low retinol with a clean inflammation marker means something quite different from a low retinol during an inflammatory response.
Now the direction that gets less attention and matters more here than for most nutrients. Because vitamin A accumulates, chronic high intake causes real harm, including liver damage, bone effects and raised intracranial pressure. High dose preformed vitamin A is also specifically contraindicated in pregnancy because of birth defect risk.
Beta-carotene from food behaves differently, since conversion is regulated and excess mostly turns your skin orange rather than causing toxicity. The concern is preformed vitamin A from supplements, cod liver oil and liver.
So this is a test worth running before supplementing rather than after, and it is one of the few where the high result is as informative as the low one.

Who this test is worth running for
Anyone taking high dose preformed vitamin A, cod liver oil, or eating liver regularly, since those are the routes to accumulation and the number is what tells you where you have landed.
Anyone with a fat malabsorption problem, including coeliac disease, inflammatory bowel disease, pancreatic insufficiency, cystic fibrosis, or after bariatric surgery. Vitamin A is fat soluble, so anything impairing fat absorption impairs it, and the same applies to vitamin D3.
Anyone with difficulty seeing in low light, persistent dry eyes, dry rough skin, or frequent infections. And anyone pregnant or planning pregnancy who is taking a supplement containing preformed vitamin A, where knowing the number matters in the upward direction.
Ordering restrictions
The lab network cannot issue orders to residents of New York, New Jersey, Rhode Island, or Hawaii. New York, New Jersey, and Rhode Island prohibit direct-to-consumer lab testing outright, requiring a treating provider's signature on every requisition. Hawaii is excluded by the network. This is tied to where you live, not where the sample is collected, so it applies to kits as well as draws.

What to do with the result
Check hs-CRP before concluding a low result means low stores. Inflammation lowers serum retinol independently of your vitamin A status, and that is the most common reason for a misleading low.
If it is genuinely low, look at fat absorption as well as intake. Vitamin A is fat soluble, so a shortfall in someone eating adequately points upstream at the gut.
If it is high, that is a real finding rather than a harmless one. Take it to a doctor, review every supplement containing preformed vitamin A, and do not simply keep taking them.
If you are pregnant or planning pregnancy, be particularly careful with the upward direction. High dose preformed vitamin A carries birth defect risk, and that is a conversation for an obstetric provider.
Do not supplement high dose vitamin A on the basis of symptoms alone. It accumulates, which makes guessing genuinely riskier here than with a water soluble vitamin.
