Joint aches
Stiff in the morning, sore by evening, and told it is just your age.
The most useful split in joint pain is inflammatory versus mechanical, and morning stiffness duration separates them. Stiffness lasting over thirty to sixty minutes, in small joints, on both sides, with visible swelling, points toward an inflammatory process that needs assessment within weeks rather than months. Stiffness easing within a few minutes of moving points toward mechanical or degenerative causes, where thyroid, vitamin D3 and inflammation markers are worth checking.
Start with the clock in the morning
Tomorrow morning, before you do anything else, notice one thing.
How long does the stiffness last?
Not how bad it is. How long until you move normally.
If you are loose within five or ten minutes of getting up and moving, note that. If you are still stiff an hour later, note that too, because those are two different situations.
Then over the next week, note two more things. Which joints, and are they the same on both sides?
That is the whole first assessment, and it costs nothing but attention.
I am putting this at the very top rather than at the bottom because of what comes next, and it is the one part of this page I would ask you not to skim.
The window that closes
Here is the fact that should get your attention, and it is the reason this page is shaped differently from the others on this site.
Inflammatory arthritis responds far better to treatment started early. There is a recognised window, measured in months rather than years, where intervention meaningfully changes how much permanent joint damage eventually occurs.
Miss that window and the damage that accumulates does not come back. That is not true of most things I write about here, where a delayed answer costs you time and comfort but not tissue.
So while nearly every other page on this site says take your time, none of this has to happen this week, this one has an exception written into it.
If your pattern is inflammatory, morning stiffness over an hour, swelling you can see, small joints on both sides, that belongs in front of a doctor within weeks. Not after a three month supplement trial. Not after an elimination diet.
I would rather lose you to a rheumatologist right now than have you spend a year optimising your omega-3 intake while something treatable does permanent work.
Everything below assumes you have made that distinction first.
The causes, and what separates each
Read this as a map rather than a diagnosis. The first item is the one to rule in or out before considering the rest.
An inflammatory process. Distinguishing signs: morning stiffness over thirty to sixty minutes, improvement with movement and worsening with rest, symmetrical small joints, visible swelling or warmth. Tests your doctor will consider: hs-CRP and ESR, plus rheumatoid factor and anti-CCP antibodies, which are diagnostic rather than screening tests.
Osteoarthritis and mechanical wear. The most common cause of joint pain overall. Distinguishing signs: stiffness under thirty minutes, worse with use and better with rest, often asymmetrical, in weight-bearing joints or hands. Levers: strength training around the joint, weight where relevant, and physiotherapy.
Thyroid, particularly underactive or autoimmune. Associated with aching, stiffness and carpal tunnel symptoms, and autoimmune thyroid disease frequently coexists with other autoimmune conditions. Distinguishing markers: a full thyroid panel with free T3, free T4 and especially antibodies.
Vitamin D3 shortfall. Associated with musculoskeletal aching and muscle weakness, and severe deficiency causes genuine bone pain. Distinguishing marker: vitamin D3. Common, cheap and correctable.
Systemic inflammation from another source. Excess visceral fat, poor sleep and insulin resistance all raise background inflammation. Distinguishing marker: hs-CRP, which tells you inflammation is present rather than where it comes from.
Gout. Sudden severe pain, often the base of the big toe, with redness and extreme tenderness. Distinctive enough that it is usually recognisable, and it needs proper treatment.
Deconditioning. Joints supported by weak muscle carry more load. Distinguishing sign: it crept in alongside a drop in activity. Fix: progressive strength training, which is one of the better interventions for joint pain generally.
The inflammation marker, and what it can do
One marker is worth a paragraph of its own here, because it does something useful and it also gets asked to do more than it can.
hs-CRP measures low-grade systemic inflammation. It is inexpensive, widely available and genuinely informative, and it is a reasonable thing to have on a panel when joints ache.
What it tells you is whether an inflammatory process is running somewhere. That is a real and useful branch point, because it separates a picture where inflammation is present from one where it is not.
What it cannot tell you is where. A raised hs-CRP could be joints, could be gut, could be an infection you have not noticed, could be visceral fat quietly generating background inflammation.
It is a smoke alarm rather than a map. Worth having, and not a diagnosis.
The other thing worth understanding is that a normal hs-CRP does not rule out inflammatory arthritis. Some people with genuine inflammatory disease have normal inflammatory markers, which is exactly why the clinical pattern, the stiffness duration and the joint distribution, carries more weight than the blood test here.
That is unusual on this site, where I usually argue for measuring rather than assuming. On this page the pattern beats the panel, and it is free.
Where the honest hedge goes
I am not a doctor and I am not diagnosing anyone, and joint pain is the area on this site where that matters most.
I want to be direct about what this page is for. It is for helping you recognise which kind of joint pain you have and take the right one to the right person quickly. It is not a treatment plan, and I would be doing you harm if I framed it as one.
The nutritional and lifestyle levers here are genuine support: omega-3 intake, reducing refined carbohydrate, strength training, correcting a real vitamin D3 shortfall, managing weight where it is loading a joint. All worth doing, all helpful, none of them a substitute for diagnosis when the pattern is inflammatory.
See a doctor promptly for joint pain with swelling, redness or warmth, morning stiffness over an hour, fever, an unexplained rash, unintended weight loss, or if a single joint becomes suddenly severely painful and hot, which can indicate infection and is urgent.
I am not saying your thyroid or your vitamin D3 is the answer. I am saying they are variables worth checking, after the more important question of what kind of pain this is has been answered.
So. Here is what I would actually do.
Tomorrow morning, at no cost. Time the stiffness. How many minutes until you move normally. Then over a week, note which joints and whether it is symmetrical.
If the pattern is inflammatory, book the appointment now. Over an hour of stiffness, visible swelling, small joints on both sides. That is the one item on this site I would not put off, for the reason in the section above.
If the pattern is mechanical, start with strength. Progressive strength training around the affected joints is one of the better-supported interventions there is, and it is available without a prescription.
When you test, include the inflammation and thyroid markers. hs-CRP, a full thyroid panel with antibodies, and vitamin D3.
Work the dietary levers as support. More omega-3, less refined carbohydrate and processed food. Genuine help, not a treatment.
Address weight where a joint is carrying it. Load through the knee multiplies body weight several times over, so this is mechanics rather than judgement.
The bigger project. A good functional medicine doctor for the systemic picture, and a rheumatologist if the pattern is inflammatory. Those are different appointments and you may want both.
This is the one page where I will not tell you it can wait. If the morning stiffness runs over an hour, make the call this week.
You are not simply wearing out. You are a set of joints reporting something specific, and the report has a shape, and the shape is readable before any test is run.
Your body is not broken. It is blocked. But on this one, some blocks close a window behind them, so this is the page where I want you moving rather than reading.
