Copper
The mineral zinc quietly displaces, measured by two tests that do not always agree.
Copper status is conventionally assessed by serum copper and ceruloplasmin together, since most circulating copper is bound to ceruloplasmin. A 2024 study of 110 healthy women compared five copper biomarkers and found ceruloplasmin, ceruloplasmin oxidase activity and total copper correlated well, while the non-ceruloplasmin-bound copper pools appeared to be differently regulated, making them complementary rather than redundant measures.
What it actually measures
Copper is required for a set of enzymes doing jobs that look unrelated until you notice what they have in common.
It is needed to mobilise iron for making red blood cells, which is why copper deficiency produces anaemia that does not respond to iron. It is part of the enzyme that builds and cross-links collagen, which is why connective tissue is affected. It is central to the final step of energy production in your mitochondria. And it is required for the enzyme that maintains the myelin sheath around nerves.
That last one is why copper deficiency produces neurological damage, and why some of that damage does not fully reverse.
Most copper in your blood, roughly 90 percent, is bound to a protein called ceruloplasmin. The rest circulates loosely bound to albumin and other carriers.
That is why the conventional assessment is two tests rather than one: total serum copper, and ceruloplasmin, read together.
What the biomarkers actually show
A 2024 observational study in the Journal of Trace Elements in Medicine and Biology examined this properly, measuring five copper biomarkers in 110 healthy women at four time points across 24 weeks.
It compared the conventional markers, total serum copper and ceruloplasmin, against three emerging ones: ceruloplasmin oxidase activity, exchangeable copper and labile copper.
Ceruloplasmin, ceruloplasmin oxidase activity and total copper correlated well with each other, followed by exchangeable copper. The labile copper pool was unrelated to the others.
The authors concluded that the non-ceruloplasmin-bound copper species represent pools that are differently regulated from total copper or ceruloplasmin-bound copper, making them interesting complementary biomarkers for a more complete assessment.
The practical translation is straightforward: the standard two tests describe one pool well, and there is more to copper status than they capture.
There is a second complication that matters more in practice. Ceruloplasmin is an acute phase reactant, meaning it rises with inflammation. So an infection or a chronic inflammatory condition can lift both ceruloplasmin and total serum copper, potentially masking a genuine deficiency.
That is the same problem ferritin has, running in the same direction, and it has the same solution: read it alongside hs-CRP.
Copper is also raised in pregnancy and by oestrogen including oral contraceptives, which is worth knowing before interpreting a high result.
What moves it
Down: sustained high-dose zinc supplementation, which is the most common cause in people who are otherwise well and the reason this page exists. Bariatric surgery and other malabsorption. Coeliac disease. Rarely, very low dietary intake. And in a genetic condition called Menkes disease.
Up: inflammation, which raises ceruloplasmin and therefore total copper independent of status. Pregnancy and oestrogen including oral contraceptives. Liver disease. And Wilson disease, a genetic condition of copper accumulation, though that typically shows a distinctive pattern of high copper with low ceruloplasmin.
The honest hedge

The reason copper deserves a page is not that deficiency is common. It is that the most frequent cause of it in otherwise healthy people is something they did to themselves on purpose.
Zinc and copper compete for absorption, and sustained high-dose zinc induces a binding protein that traps copper in the intestinal lining. A 2005 report in the American Journal of Clinical Pathology described three cases of zinc-induced copper deficiency identified initially on bone marrow examination, and a 2023 case report in Cureus documented it presenting as neurological deficit and anaemia.
Those are case reports, so they establish that this is real and can be serious rather than telling you how often it happens. Most people taking zinc do not develop copper deficiency.
What makes it worth attention is that the blood findings correct with copper and the neurological ones sometimes do not.
The practical caution: if you have been taking high-dose zinc for months or years without copper, and you have unexplained anaemia, a low white cell count, or new numbness, tingling or balance problems, that is a doctor now rather than a supplement adjustment.
And do not supplement copper on a guess. It has a narrow useful range and excess is genuinely harmful.
So. What to actually do.
Tonight, at no cost. Add up your total zinc across every product you take, and check whether any of them contain copper. Zinc hides in multivitamins, immune formulas and prostate blends.
If you take high-dose zinc alone long term, that is the finding. It is not a reason to panic and it is a reason to stop taking it in isolation.
Ask for both tests together. Serum copper and ceruloplasmin, since either alone is harder to interpret.
Add an inflammation marker. hs-CRP tells you whether a comfortable copper result can be trusted, since ceruloplasmin rises with inflammation.
Run a complete blood count alongside. A CBC matters because anaemia and low neutrophils are usually what show first in copper deficiency.
