Vitamin B12

The one where waiting to be sure costs more than being wrong.

Serum B12 is the standard test, with most labs using a reference range starting around 200 pg/mL. Neurological symptoms of B12 deficiency are documented occurring at levels inside that range and without any blood abnormality, which the literature calls functional deficiency. Methylmalonic acid is the more sensitive confirmatory test when a result sits in the borderline zone.

What it actually does

B12 has two jobs that matter here, and they fail on completely different timescales.

The first is helping make red blood cells. When B12 runs short, that process falters and you eventually get anemia. That is the version most people know about, and it is the version a standard blood count would eventually catch.

The second is maintaining myelin, the insulating sheath around your nerves. Without enough B12 that insulation degrades, and nerve signalling degrades with it. Numbness, tingling, balance problems, memory difficulty.

Here is the part that matters. Those two do not fail in a fixed order. Neurological damage can be well underway while the blood count still looks entirely normal, and in a meaningful proportion of people the nerve symptoms arrive first.

Which means waiting for anemia to confirm a B12 problem is waiting for the wrong signal, and waiting has a cost.

The normal result that is not reassuring

Here is where I have to tell you something that reframes a normal number, so listen close.

Neurological symptoms of B12 deficiency are documented occurring at serum levels inside the standard reference range, with no blood abnormality at all. The literature has a name for it: functional B12 deficiency, where activity inside the cell is insufficient even though the concentration in the blood looks adequate on paper.

There is a related finding worth knowing: metabolic disturbances consistent with insufficiency have been detected at B12 levels as high as 400 pg/mL, which is comfortably inside what any lab would call normal.

So the bottom third of the reference range is not a safe place to sit and conclude the question is closed, particularly with symptoms present.

Those are two completely different things, a level inside the range and a level that is enough for you, and the gap between them is where a lot of nerve damage quietly accumulates.

The test that settles it

When serum B12 comes back in that borderline zone, roughly 150 to 400 pg/mL, and there are symptoms, the standard next step is methylmalonic acid.

The logic is straightforward. Methylmalonic acid is a substance that accumulates when B12 activity inside cells falls short. It rises because the cellular process is failing, which makes it a measure of function rather than of concentration. It frequently rises before serum B12 drops below the conventional cutoff.

So a borderline B12 with a raised methylmalonic acid points at genuine functional deficiency. A borderline B12 with a normal methylmalonic acid is considerably more reassuring than the B12 number alone.

That is the test to ask for by name if you are in the middle zone and something is not right.

Why timing matters more here than almost anywhere

Most nutrient shortfalls are patient. Correct them late and you mostly catch up.

B12 is not like that. Treatment appears considerably more effective when cognitive symptoms have been present for less than a year than when they have run longer, and neurological damage from prolonged deficiency may not fully reverse.

That is the whole argument for asking the question early rather than waiting to see whether it passes. The blood picture will recover. The nerves may not, if you leave it long enough.

I would rather be direct about that than gentle, because the practical implication is simple: if you have numbness, tingling, balance trouble or memory difficulty and you are in a risk group, this is worth raising now rather than at your next annual visit.

What moves it

Down: eating little or no animal food, since B12 is essentially absent from plants. Age, as stomach acid production falls and absorption falls with it. Metformin, taken long term. Acid-suppressing medication, particularly proton pump inhibitors. Gut conditions affecting absorption, including celiac and Crohn's disease. Gastric or intestinal surgery. Pernicious anemia, where an autoimmune process blocks absorption directly. Heavy alcohol use. Nitrous oxide exposure, which inactivates B12 and is a genuine and underappreciated cause.

Up: animal foods, particularly liver, red meat, fish, eggs and dairy. Fortified foods. Supplementation, which works well orally for most people, and by injection where absorption itself is the problem.

The honest hedge

I am not a doctor and I am not diagnosing anyone.

There is one thing on this page I would treat as a genuine safety point rather than general information. Folate supplementation can correct the anemia of B12 deficiency while leaving the neurological damage to progress unchecked, which is why the two are measured together and why supplementing folate alone in someone who has not had B12 checked is a poor idea.

Beyond that: B12 is water soluble and excess is generally excreted, so it has a wide safety margin. That is not a reason to take it blindly, because if your level is genuinely low the more useful question is why. Diet, absorption and medication have completely different answers, and only one of them is solved by a bottle.

So. What to actually do.

Tonight, at no cost. Check whether you are in any of the risk groups above. Little or no animal food, over sixty, on metformin, on long-term acid suppression, or with a gut condition. If two or more apply, this is worth measuring rather than assuming.

When you test, pair it. B12 and folate together, since they work in the same pathway and one can mask the other's consequences.

If the result lands between 200 and 400 and you have symptoms. Ask specifically about methylmalonic acid. That is the test that separates a fine number from a fine situation.

If you have neurological symptoms. Numbness, tingling, balance trouble or new memory difficulty. Raise it with a doctor now rather than waiting, because that is where the timing argument bites.

If you supplement. Methylcobalamin or hydroxocobalamin over cyanocobalamin, and if absorption is the underlying problem, discuss the route with a provider rather than assuming oral will do.

None of this has to happen this week. But tonight you can check whether you are in a risk group nobody has thought to ask about.

You are not imagining the tingling, and you are not being dramatic about the fog. You are running a nutrient that maintains the insulation on your nervous system, measured by a test that can read fine while the insulation frays.

Is B12 on a standard blood panel?
