B12 and Folate Panel
Two halves of one pathway, and testing either one alone is the mistake that matters here.
This panel measures serum vitamin B12 and folate together. They work as a pair in the same methylation pathway and produce the same blood picture when deficient, and correcting folate while an unidentified B12 shortfall continues can mask the anaemia while neurological damage progresses. That is why they are tested together rather than one at a time.
What this test actually measures
B12 and folate are usually described as two vitamins. It is more useful to think of them as two halves of one machine.
Both are required at the same step in a process called methylation, which is the business of attaching small chemical tags to things in order to switch them on or off. Your body uses it to build neurotransmitters, maintain DNA, and process hormones, billions of times a second.
They also both matter for making red blood cells, which is why a shortfall in either produces the same picture on a blood count: red cells made larger and fewer, because the cell keeps growing while waiting for its DNA copy to finish.
That shared signature is exactly why they are ordered together. A blood count showing large red cells cannot tell you which of the two is short, and getting that wrong has consequences.

What a normal result does and does not settle
Start with B12, because it has a specific and well documented interpretive problem.
Neurological symptoms of B12 deficiency are documented occurring at serum levels inside the standard reference range, and without the blood changes that usually accompany a deficiency. The literature describes this as functional B12 deficiency: the amount in circulation looks adequate while cellular activity is not.
So a B12 result in the lower part of normal is a conversation rather than a closed question, particularly if you eat little animal food, take metformin or long term acid-suppressing medication, or are over sixty.
There is a timing element that makes this urgent rather than academic. Treatment appears considerably more effective when cognitive symptoms have been present for less than a year than when they have run longer, and some neurological damage does not fully reverse. That is an argument for asking early rather than waiting to see whether it passes.
Folate has a different interpretive wrinkle. Serum folate responds within days to what you have eaten, so a few good salads before the appointment can lift it. Red cell folate reflects the previous three to four months and is the better measure of actual status where it is available.
Now the reason these two share a page. In the 1940s and 50s high dose folic acid was used to reverse the anaemia of B12 deficiency. It worked on the blood count while the underlying B12 deficiency continued and its nerve damage progressed, with the warning sign switched off.
A 2024 review in Food and Nutrition Bulletin revisited that for an era of mandatory fortification and widespread supplements. It reported that cognitive test scores are lower, and homocysteine and methylmalonic acid higher, in people with low B12 and elevated folate compared with low B12 and non-elevated folate. The review was careful that much of this evidence is correlative and the mechanism untested, while concluding it warrants increased vigilance.
The practical consequence is simple and it is why this is one order rather than two: test both, then correct what is actually short.

Who this test is worth running for
Anyone with unexplained fatigue, brain fog, tingling or numbness in hands or feet, balance problems, low mood, or a blood count showing enlarged red cells.
Specifically worth running if you eat little or no animal food, take metformin, take long term acid-suppressing medication, are over sixty, have had bariatric surgery, have coeliac or inflammatory bowel disease, or drink heavily. Every one of those affects absorption or intake of one or both.
It is also worth asking your provider about homocysteine alongside, which is the functional read on whether the pathway that depends on both vitamins is actually running. Serum levels tell you what is present, homocysteine tells you whether it is working, and the two answer different questions.
Ordering restrictions
The lab network cannot issue orders to residents of New York, New Jersey, Rhode Island, or Hawaii. New York, New Jersey, and Rhode Island prohibit direct-to-consumer lab testing outright, requiring a treating provider's signature on every requisition. Hawaii is excluded by the network. This is tied to where you live, not where the sample is collected, so it applies to kits as well as draws.

What to do with the result
If B12 is low, or low-normal with symptoms, take it to a provider rather than starting a supplement, because the why matters. Absorption problems, pernicious anaemia and medication effects all need different responses, and the route of replacement depends on the cause.
If folate is low, check B12 has been seen first. Correcting folate while an unidentified B12 shortfall runs underneath is the specific pattern this page exists to prevent.
If folate is high and B12 is low or borderline, that is the combination the 2024 review flagged, and it deserves a proper conversation rather than more of either.
If both look adequate but symptoms persist, ask your provider about adding homocysteine, which is a functional read the serum levels cannot give, and look at the other causes on the brain fog list rather than assuming these two were the answer.
