Fasting Insulin Test
The marker that moves years before glucose does, and the one your annual physical has never run.
A fasting insulin test measures how much insulin your body is producing after an overnight fast, which is a direct read on how hard your pancreas is working to hold blood sugar steady. Because insulin rises for years to compensate for reducing cell sensitivity, it moves long before fasting glucose or HbA1c do, and it is not included on any standard annual panel.
What this test actually measures
Every standard annual panel measures your blood glucose. Almost none measure the hormone that is holding it there.
Insulin is what your pancreas releases to move glucose out of your bloodstream and into your cells. When your cells respond well to it, a small amount does the job. When they respond less well, your pancreas compensates by producing more.
So a fasting insulin measurement is a read on effort rather than on result. It tells you how much hormone it is currently taking to keep your overnight blood sugar where it belongs.
That distinction is the entire value of the test. Two people can have identical fasting glucose readings, with one producing three times as much insulin to achieve it, and only one of them is heading somewhere.
Fasting matters here in a way it does not for HbA1c, because insulin responds within minutes to food. This is a snapshot, and it has to be taken at a standardised moment to mean anything.
The years this test can see and the others cannot
Here is the sequence that makes this the most useful unordered test in metabolic health.
Your cells become gradually less responsive to insulin. Your pancreas notices glucose drifting up and produces more insulin. More insulin does the job. Glucose comes back down. Everything reads normal.
That compensation can run for years. Fasting glucose stays under 100. HbA1c stays under 5.7. Every annual physical returns the same reassuring sentence, and it is entirely accurate.
Glucose only starts to rise when the compensation begins to fail. Which means a rising glucose is a late signal, and a normal glucose tells you the compensation is still working rather than that nothing is happening.
Fasting insulin sees the compensation itself. That is the whole point of ordering it.
On numbers, I want to be honest about where the ground is firm and where it is not. Most labs report a reference range roughly from 2 to 25 µIU/mL, which is wide enough to include a great deal of significant insulin resistance. Practitioners working functionally generally aim below about 5 to 7, and that target comes from practitioner consensus and physiological reasoning rather than from outcome trials with a defined cutoff.
What is better established is the direction and the company it keeps. A 2026 systematic review of 40 studies found progression to diabetes was predicted considerably better by combinations of tests than by any single one, and a 2024 review of 32 studies covering 49,782 participants supported the triglyceride to HDL ratio as a simple accessible marker of insulin resistance, reporting average cutoffs around 2.53 for women and 2.8 for men.
So the sensible use is not to chase an exact insulin number. It is to find out whether the compensation is running at all, and to read it alongside HbA1c and your lipid ratio.
Test details
What is measured: serum insulin after an overnight fast, usually in µIU/mL.

How the pricing works
I want to be straightforward about this, because lab pricing in the wellness industry frequently is not.
Jess takes no markup on the test itself. The lab cost is passed through exactly as it comes. What you pay is the test cost, the dispensary's service fee, the authorization fee, the collection fee where one applies, a flat handling fee for placing and managing the order, and the card processing cost. That structure is deliberate. A percentage markup on labs creates an incentive to recommend expensive tests, and this way there is not one.

Who this test is worth running for
Anyone who has been told their blood sugar is fine and does not feel fine. That is the core case, because a normal glucose and a working compensation look identical on a standard panel.
Specifically: weight that will not move despite genuine effort, an afternoon crash that arrives about ninety minutes after eating, persistent sugar cravings, polycystic ovary syndrome, a family history of type 2 diabetes, excess weight around the middle, or a triglyceride to HDL ratio above about 2.5.
One practical note that will save you a wasted test. Short sleep measurably reduces insulin sensitivity, and a 2021 randomised crossover study found four nights at four hours produced hyperinsulinaemia and insulin resistance in healthy young men. If you are currently sleeping badly, you may end up measuring your schedule rather than your metabolism. Fix the sleep for a few weeks first if you can.
Ordering restrictions
The lab network cannot issue orders to residents of New York, New Jersey, Rhode Island, or Hawaii. New York, New Jersey, and Rhode Island prohibit direct-to-consumer lab testing outright, requiring a treating provider's signature on every requisition. Hawaii is excluded by the network. This is tied to where you live, not where the sample is collected, so it applies to kits as well as draws.

What to do with the result
If it comes back genuinely high, that is worth acting on and the levers are well established. Resistance training builds the tissue that stores most of your glucose, walking after meals blunts the spike, and reducing refined carbohydrate reduces the demand. Those work, and they work over months rather than weeks.
If it is high with a normal HbA1c, that is exactly the picture this test exists to reveal. Nothing is wrong on your standard panel and something is genuinely happening, and you have found it early, which is the good version of this result.
If it is low and your glucose is high, that is a different situation entirely and it belongs with a doctor promptly rather than with a lifestyle plan.
Check your sleep before drawing conclusions. If you have been sleeping five hours, retest after a few weeks of proper sleep before deciding what the number means.
Retest at about three months after a genuine change. Insulin sensitivity recovers over months, and testing sooner mostly measures noise.
