Wired but tired

Running on empty all day, and then the lights come on the moment your head hits the pillow.

Wired but tired describes exhaustion through the day paired with alertness at night. The causes worth separating are a drifted circadian rhythm, caffeine and alcohol timing, overnight blood sugar dips, thyroid function in either direction, and unrefreshing sleep including sleep apnoea. Salivary cortisol panels are a weak basis for a plan here, since the systematic review evidence behind adrenal fatigue found conflicting results across 58 studies.

Start with what time it turns

Before any list of causes, answer one question, and be specific about it.

What time does the switch flip? Is there an hour in the evening where the tiredness lifts and something like energy arrives?

For most people there is, and it is usually somewhere between nine and eleven. Knowing the hour matters, because a predictable turn points at your body clock, while a random one points elsewhere.

Now a second question. On the morning after a night you slept eight full hours, do you wake refreshed?

If the answer is no, hold onto that. It is the single most important detail on this page and it changes what you should investigate first.

Write both answers down. They narrow the list faster than any blood test.

Why nobody has found it yet

Here is the tension though.

Wired but tired does not have a lab value. There is no line on any panel that reads yes, this person is running on adrenaline at 11pm and flattened by 2pm.

So when you take it to an appointment, one of two things usually happens. Either you get a normal panel and a suggestion to manage your stress, or you get sent to a wellness site that hands you a saliva kit and a story about adrenal fatigue.

I want to deal with the second one honestly, because it is the more comfortable answer and it is the weaker one.

A 2016 systematic review in BMC Endocrine Disorders went looking for the evidence. It searched 3,470 articles, applied formal criteria, and analysed 58 studies, 33 in healthy people and 25 in symptomatic patients. It looked specifically at the tests the concept rests on: direct awakening cortisol, the cortisol awakening response, and salivary cortisol rhythm.

What it found was an almost systematic pattern of conflicting results across the studies, and it concluded there was no substantiation for adrenal fatigue as a condition.

Now, here is what that does and does not mean, because this is where the honest version differs from both of the usual answers.

It does not mean you are imagining this. The exhaustion is real and the pattern is real and you are not the first person to describe it precisely.

It means the saliva panel is not the test that will explain it, and building a supplement protocol on a cortisol curve is building on sand. Which is genuinely useful news, because it points you at the things that do have answers.

The causes, and what separates each

Read this as a map rather than a diagnosis. Several of these commonly run together, which is exactly why one change at a time so often does nothing.

Your body clock has drifted later than your schedule. This is the most common single explanation and the most fixable. Dim mornings and bright evenings push your internal clock later, so the alerting window before your own melatonin release lands right at bedtime. Distinguishing signs: a predictable turn-on hour, and feeling far better on a holiday when you can wake naturally. No blood test involved.

Caffeine and alcohol timing. Caffeine has a half life of roughly five to six hours, so half of a 3pm coffee is still circulating at 9pm. Alcohol makes you fall asleep faster and then fragments the second half of the night. Distinguishing test: move all caffeine before noon for ten days and see what changes. Free, and more informative than most panels.

Blood sugar dipping overnight. A drop in glucose triggers cortisol and adrenaline, which are alerting by design. If you wake in the small hours, or if a late carbohydrate-heavy meal reliably makes it worse, this belongs near the front. Distinguishing markers: fasting insulin alongside HbA1c.

Thyroid, in either direction. An underactive thyroid causes fatigue and unrefreshing sleep. An overactive one causes the literal wired version, with racing heart, heat intolerance and real exhaustion together. Distinguishing markers: a full thyroid panel with free T3, free T4 and antibodies, not TSH alone.

Sleep that is not restoring you. Hours in bed is not sleep achieved. Sleep apnoea produces exactly this presentation and is very often missed, particularly in women where it looks different from the textbook description. Distinguishing test: a sleep study, not a blood test. If you wake unrefreshed after a full night, put this first.

Low iron stores. Iron shortfall produces daytime exhaustion and is associated with restless legs, which fragments sleep from the other end. Distinguishing marker: ferritin, read alongside hs-CRP so an inflamed number is not mistaken for a full one.

Sustained stress load, without needing a cortisol panel to name it. If you are carrying more than you can put down, that affects sleep onset, sleep quality and blood sugar regulation all at once. You do not need a saliva test to know this is present, and addressing it is worth more than measuring it.

The sleep and blood sugar loop

Here is the part that reframes the whole pattern, so listen close.

Most people treat the tiredness and the wiredness as two problems. They are usually one loop, and it runs in a direction most people do not expect.

A 2021 randomised double-blind crossover study in the Journal of Clinical Endocrinology and Metabolism put 34 healthy young men through four nights of sleep restricted to four hours, twice, once with cortisol and testosterone held fixed by a clamp and once with placebo.

Sleep restriction alone produced hyperinsulinaemia, hyperglycaemia and overall insulin resistance. Four nights. In healthy young men.

And when the researchers clamped cortisol and testosterone so they could not move, the development of insulin resistance and hyperinsulinaemia was cut by about half.

Read what that actually demonstrates. Short sleep drives insulin resistance, and it does so substantially through the hormonal shift it causes. Which means the exhaustion is not just a consequence of the poor sleep, it is being amplified by a metabolic change the poor sleep created.

And then the metabolic change feeds back. Unstable blood sugar overnight triggers alerting hormones. Alerting hormones fragment sleep. Fragmented sleep worsens insulin resistance.

That is the loop. Which is genuinely good news, because a loop can be entered at any point. You do not have to fix the whole thing at once. You have to break one link, and the sleep timing link is usually the cheapest one to break.

Where the honest hedge goes

I am not a doctor and I am not diagnosing anyone.

I have been deliberately blunt about the adrenal fatigue evidence, and I want to be equally clear about why. Not to dismiss what you are feeling. Because a wrong map wastes months, and months matter when you are this tired.

The honest version is that most of what drives this pattern is measurable or observable, and the parts that are not measurable, your stress load and your sleep timing, are addressable without a test anyway.

There are presentations here that need a doctor rather than a routine. A persistently racing or irregular heartbeat, unexplained weight loss, heat intolerance with sweating and tremor, all point at an overactive thyroid. Heavy snoring with waking gasps points at sleep apnoea. Both need diagnosis.

And if low mood, hopelessness or significant anxiety is part of this for you, that deserves care in its own right rather than being folded into a fatigue explanation. If things ever feel unbearable, in the US you can call or text 988 for the Suicide and Crisis Lifeline, any hour.

I am not saying your thyroid or your blood sugar is the answer. I am saying they are variables, findable ones, and almost nobody has measured them properly before landing on stress as the explanation.

So. Here is what I would actually do.

Tonight, at no cost. Get outside within thirty minutes of waking, for ten minutes, without sunglasses. Morning light is the strongest signal your body clock receives and it is the single highest return change on this page. Then pick a wake time and hold it, including weekends.

This week, run one free experiment. Move all caffeine to before noon for ten days. Do not change anything else. Caffeine is the most common contributor here and it is the easiest one to rule in or out without spending anything.

Log two numbers for a fortnight. The hour the switch flips, and whether you woke refreshed. Two weeks of that shows a pattern that no single night reveals.

Shift your last meal. Adequate protein and fat at dinner rather than a late carbohydrate-heavy one. If you wake in the small hours, this is the cheapest thing to test against that.

When you test, go wide once. A full thyroid panel, ferritin with hs-CRP, vitamin D3, and fasting insulin with HbA1c. One draw, and running them together is what lets you tell them apart.

If you wake unrefreshed after a full night, ask about a sleep study. That is the investigation this symptom most often actually needs, and no blood panel substitutes for it.

The bigger project. Take the results to a good functional medicine doctor who reads them as a system rather than as a checklist.

None of this has to happen this week. But tomorrow morning you can walk outside for ten minutes before you look at a screen.

You are not badly designed. You are a clock, a fuel curve and a repair window, and right now the clock is running on a different schedule from the life around it.

Your body is not broken. It is blocked. And in this case the block is usually timing rather than damage, which means it moves faster than almost anything else on this site.

This is one reason an evening meal with adequate protein and fat, rather than a carbohydrate-heavy one late at night, often changes how people sleep. Fasting insulin and HbA1c are the markers that show whether your blood sugar regulation is under strain.
How long does it take to fix wired but tired?
Circadian timing responds surprisingly fast, often within one to two weeks of consistent morning light and a fixed wake time, because the clock is being reset rather than repaired. Caffeine and alcohol changes show up within days. Anything driven by an underlying marker, thyroid, iron or blood sugar, moves on that marker's own timeline, which is generally months rather than weeks.
Should I take melatonin for wired but tired?
It is worth discussing with a provider, and the timing matters more than most people realise. Melatonin used as a circadian signal, taken in a small dose several hours before target bedtime, works on a different principle from melatonin used as a sedative at bedtime, and the first use is where the stronger case sits. Because it is a hormone rather than a herb, dose and duration are worth deciding with someone rather than escalating on your own.

Why do I get a second wind at 10pm?
Your cortisol follows a daily curve, high in the morning and falling through the day, and your body also produces a natural alerting signal in the couple of hours before its own melatonin release. If your internal clock is running late, that alerting window lands right when you are trying to wind down. Working past it into the small hours is what makes the following day worse, which is how the loop sustains itself.
