Cortisol Test
A single point on a curve that moves more across a day than most hormones move in a lifetime.
Cortisol is your primary stress hormone, produced by the adrenal glands and following a strong daily rhythm: highest shortly after waking, falling through the day, lowest around midnight. A single blood cortisol is therefore one point on a steep curve and must be drawn at a standardised time, conventionally early morning. It is a diagnostic test for specific adrenal conditions rather than a useful measure of everyday stress.
What this test actually measures
Cortisol has a reputation as the stress hormone, which is true and slightly misleading, because most of what it does has nothing to do with stress.
It regulates blood sugar, blood pressure, immune activity and inflammation, and it is central to how you wake up. Without it you could not function.
The defining feature for anyone ordering this test is its rhythm. Cortisol peaks sharply within about half an hour of waking, falls steeply through the morning, continues declining through the day, and reaches its lowest point around midnight.
That swing is large. A perfectly healthy person can have a morning cortisol several times their evening value, and both are entirely normal.
So a single blood cortisol is one point plotted on a steep curve. Which is why the time of the draw is not a detail here, it is most of the interpretation.
This test measures total cortisol, meaning both the protein-bound majority and the small free fraction that is biologically active.
What this test is genuinely for
Let me be direct about scope, because cortisol is one of the most over-ordered and over-interpreted tests in wellness.
A morning serum cortisol is a real diagnostic test with real uses. It is part of the workup for Cushing's syndrome, where cortisol is genuinely too high, and for adrenal insufficiency, where it is genuinely too low. Both are serious conditions and both are properly diagnosed with dynamic testing that this single measurement feeds into.
What it is not is a measure of how stressed you have been. A raised morning cortisol in someone under pressure at work is not a finding, it is Tuesday.
There is a whole category of interpretation built around cortisol curves, usually salivary, offered as evidence of adrenal fatigue. A 2016 systematic review in BMC Endocrine Disorders searched 3,470 articles and analysed 58 studies looking specifically at direct awakening cortisol, the cortisol awakening response and salivary cortisol rhythm. It found conflicting results almost systematically across those studies and concluded there was no substantiation for adrenal fatigue as a condition.
I want to be precise about what that does and does not say, because it is easy to hear it as dismissal.
It does not say stress has no physical effects. It plainly does. A 2021 randomised crossover study found that clamping cortisol and testosterone during four nights of short sleep cut the resulting insulin resistance roughly in half, which is direct evidence that cortisol changes have real metabolic consequences.
What the review says is narrower: the specific cortisol testing used to diagnose adrenal fatigue does not produce consistent findings, so building a treatment plan on a cortisol curve is building on unreliable ground.
So the honest position is that cortisol matters, and measuring it in a single blood sample tells you far less about your life than it appears to.

How the pricing works
I want to be straightforward about this, because lab pricing in the wellness industry frequently is not.

What you pay is the test cost, the dispensary's service fee, the authorization fee, the collection fee where one applies, a flat handling fee for placing and managing the order, and the card processing cost. That structure is deliberate. A percentage markup on labs creates an incentive to recommend expensive tests, and this way there is not one.

Who this test is worth running for
Anyone whose doctor is investigating a specific adrenal condition. That is the real indication, and in that context this test is one input into a proper workup rather than a standalone answer.
Specifically worth it where there are features suggesting Cushing's syndrome, including rapid central weight gain with purple stretch marks, easy bruising, a rounded face and muscle weakness, or features suggesting adrenal insufficiency, including profound fatigue, low blood pressure, salt craving, weight loss and skin darkening. Both belong with a doctor promptly.
It is a poor first test for ordinary fatigue or stress. If that is your question, a full thyroid panel, ferritin with hs-CRP, B12 and folate, vitamin D3 and fasting insulin answer considerably more from the same draw, and a sleep study answers more still if you never wake refreshed.
Ordering restrictions
The lab network cannot issue orders to residents of New York, New Jersey, Rhode Island, or Hawaii. New York, New Jersey, and Rhode Island prohibit direct-to-consumer lab testing outright, requiring a treating provider's signature on every requisition. Hawaii is excluded by the network. This is tied to where you live, not where the sample is collected, so it applies to kits as well as draws.

What to do with the result
Check the time of your draw before anything else. A cortisol result without a recorded time is close to uninterpretable, given how steeply the curve falls.
If it is markedly high or markedly low, that belongs with a doctor. Both directions point at conditions that need dynamic testing to confirm, and neither is a wellness project.
If it is unremarkable and you are exhausted, that is not a dead end. It means cortisol is not the answer, which is genuinely useful, and it points you at the markers that more often are.
Do not build a supplement protocol on a cortisol value. The systematic review evidence does not support it, and the same money spent on a sleep study or a thyroid panel does more work.
Address the stress load regardless of the number. It affects sleep, blood sugar and recovery whether or not a test confirms it, and it does not require a lab result to be worth working on.
